Oregon Yellow Fever Program

Certification and administration site registration

Providers interested in administering yellow fever vaccination must apply for Oregon Yellow Fever Program certification and register their administration sites (pharmacies, clinics, hospitals, etc.). Registration allows providers to purchase and store yellow fever vaccines and related supplies.   


Providers are responsible for ordering and purchasing their own yellow fever stamps. OHA will provide all necessary information to ensure that purchased stamps meet international standards for recognition and verification of yellow fever vaccination. The stamp allows certified providers to sign the “yellow card," which validates the record.

Eligibility

 

Credentialed health care providers eligible to provide yellow fever vaccine:

  • Clinical Nurse Specialist (CNS)
  • Dentist (DDS/DMD)
  • Medical Doctor (MD)
  • Nurse Practitioner (NP)(with prescription-writing privileges)
  • Naturopath Doctor (ND)
  • Osteopathic Doctor (DO)
  • Pharmacist
  • Physician’s Assistant (PA)

 The stamp can move with the vaccinator but not across state lines. It is only approved for Oregon.

How to apply for yellow fever certification

  1. Complete the required CDC Yellow Fever Vaccine: Information for Health Care Professionals Advising Travelers Course.
  2. Complete the application form in full, upload the CDC Yellow Fever Course Completion Certificate to the form, acknowledge the yellow fever requirements and submit the application form.
  3. Re-certify as an Oregon yellow fever provider every three years. Certified providers will be notified 15 and 30 days prior to their renewal date. Failure to renew voids eligibility to participate in Oregon's yellow fever program.

Please allow for a processing time of 1-2 weeks for approval.

Adverse event reporting

Online reporting is strongly encouraged. Please report clinically important adverse events that occur after vaccination of adults and children, even if you are not sure whether the vaccine caused the adverse event.

The Vaccine Adverse Events Reporting System (VAERS) accepts all reports, including reports of vaccination errors.
