CHA and CHP learning collaborative resources

From March 2022 to June 2023 the OHA Transformation Center hosted a virtual community health assessment (CHA) and community health improvement plan (CHP) learning collaborative. This eight-session learning series focused on the development of a collaborative CHA and CHP. This learning collaborative was facilitated by the Providence Center for Outcomes Research and Education (CORE).
