Applying for Certification

Requirements for Initial Applicants:

To obtain Temporary Certification, psychiatrists, licensed psychologists, and licensed clinical social workers (LCSW)* pursuing certification as a Forensic Evaluator must submit the following materials:

• Completed application (form below)
• Supplement (form below)
• Proof of current Oregon license to practice
• Curriculum vitae or résumé 
• $250 nonrefundable application fee

To obtain Full Certification, initial applicants must also:

• Submit three redacted competency and/or criminal responsibility forensic evaluations for review and approval.
  
• Successfully complete the multi-day Oregon Forensic Evaluator initial training.
  

*LCSWs are restricted to performing forensic evaluations related to a youth's fitness to proceed.
