Amending an Original Record of Live Birth Prior to Adoption


In-person customer assistance to change vital records is not available.


Estimated amendment order processing times are currently averaging (6) months. 

  • If you submitted a request and processing fee, the date your check or money order is cashed represents the verification of receipt. It also helps you estimate the time remaining before your request is processed. 
  • Requests are reviewed and processed in the order they were received. If there are any issues with your request, you will be contacted at that time.

Amending an Original Record of Live Birth Prior to Adoption

A person may request a biological parent name be added to or changed on the original birth record prior to an adoption. The applicant is the person requesting to have their biological parent name be added or changed.

The applicant must be:

  • The person whose current record of live birth after adoption is filed with the Oregon Center for Health Statistics;
  • The person who is listed on the original record of live birth prior to adoption;
  • Age 21 or older; and
  • The person who has a court order to open the sealed file under ORS 432.250 or has a copy of their original record of live birth issued under ORS 432.228.

The following documents and fee are required to be provided:


    1    . Application in Support of Amending an Original Record of Live Birth Prior to Adoption.
    2. A copy of the court order used to open the sealed file containing the original record of live birth prior to adoption, if applicable.
    3. Evidence that the parent to be added is the biological parent, including but not limited to:
    a. A DNA test identifying that the person whose name is to be entered as the biological parent is the biological parent of the applicant;
    b. A copy of an order from a court of competent jurisdiction identifying that the person whose name is to be entered as the biological parent is the biological parent of the applicant; or
    c. Other official documents acceptable to the state registrar.

        The amended original record of live birth will be mailed to the applicant's mailing address provided on the application.
        OAR 333-011-0277 Application and Documentation in Support of Amending an Original Record of Live Birth Record, allows adopted individuals who are 21 years of age and older and have had their original birth records unsealed to amend that birth record to add the name of a biological parent if parentage is confirmed by DNA testing. Although this amendment will be recorded by the Center for Health Statistics, it does not confer any new rights or responsibilities upon any of the named parties.
