What to Expect at an Appointment

When you go to a health care appointment, your provider may ask you questions about your demographic information. They might ask you these questions verbally, or give you a form to complete. 

Why We Ask

The Oregon Health Authority (OHA) wants to make sure that by 2030, everyone in Oregon can get the healthcare they need. We use information about race, ethnicity, language, disability (REALD), and sexual orientation and gender identity (SOGI) to help us know what communities need. We use this information to decide how to use resources in the best way possible to help everyone. Getting this information helps us meet the needs of communities who have been ignored or treated unfairly in the past.
  • Your provider must ask you all of the REALD & SOGI questions, but you do not have to answer all of them.
  • Every question includes answer options such as "Don't know" and "Don't want to answer".
