Community information exchange support for HRSN providers
OHA has contracted with HIT Commons to provide support and training for HRSN service providers using Connect Oregon/Unite Us community information exchange (CIE). This support is available from 2026–2027 and is for current HRSN service providers and organizations interested in becoming providers.
HIT Commons is working with CCOs to ensure this work complements their efforts. Support for HRSN providers can include direct training and technical assistance, regional workshops, data use support, and more.

Getting started as an HRSN provider
OHP Open Card/fee-for-service provider enrollment
- In this short, pre-recorded training, we show new providers how to fill out the application to become an HRSN service provider with OHP Open Card/fee-for-service and existing providers how to add on HRSN specialties.

Coordinated care organization provider enrollment
- If you also plan to enroll as an HRSN service provider with a coordinated care organization (CCO), please contact the CCO directly to learn more about their processes.
HRSN provider workflows
- In this pre-recorded module, you’ll hear from three HRSN service providers as they walk through their organization’s process for delivering HRSN housing services. They go over their internal systems for tracking information, handling referrals, and case management.
