Art Therapy Program - License Information

How to get licensed - Art Therapist

 An individual applying for initial licensure as an Art Therapist must:
 
  • Meet the requirements of Oregon Administrative Rule 331, Division 30;
  • Submit a completed application form found in forms below;
  • Submit one form of original photographic identification. Acceptable identification must include applicant's current legal name. Front and back of legible (clear) photocopies if submitted by mail. Acceptable identification options can be found under Chapter 331, Division 30 of Oregon Administrative Rule;
  • Be at least 18 years of age;
  • Have received at least a master's degree from a program accepted by the Art Therapy Credentials Board, Inc. (ATCB);
  • Arrange for official documentation to be sent directly from the ATCB to the Health Licensing Office showing proof that the applicant meets the requirements to be certified as a Registered Art Therapist (ATR) or possess the qualifications to be credentialed as an ATR.
  • Pass a fingerprint-based nationwide criminal records check; and
  • Pay all required fees.

How to get licensed - Certified Art Therapist

 An individual applying for initial licensure as a Certified Art Therapist must:
 
  • Meet the requirements of Oregon Administrative Rule 331, Division 30;
  • Submit a completed application form found in forms below;
  • Submit one form of original photographic identification. Acceptable identification must include applicant's current legal name. Front and back of legible (clear) photocopies if submitted by mail. Acceptable identification options can be found under Chapter 331, Division 30 of Oregon Administrative Rule;
  • Be at least 18 years of age;
  • Have received at least a master's degree from a program accepted by the Art Therapy Credentials Board, Inc. (ATCB);
  • Arrange for official documentation to be sent directly from the ATCB to the Health Licensing Office showing proof that the applicant either meets the requirements as a Board Certified Art Therapist (ATR-BC) by the date of application; or possesses the qualifications to be credentialed as an ATR-BC.
  • Pass a fingerprint-based nationwide criminal records check; and
  • Pay all required fees.

How to get licensed - Provisional Art Therapist

An individual applying for initial licensure as a Provisional Art Therapist must:
 
  • Meet the requirements of Oregon Administrative Rule 331, Division 30;
  • Submit a completed application form found in forms below;
  • Submit one form of original photographic identification. Acceptable identification must include applicant's current legal name. Front and back of legible (clear) photocopies if submitted by mail. Acceptable identification options can be found under Chapter 331, Division 30 of Oregon Administrative Rule;
  • Be at least 18 years of age;
  • Have received at least a master's degree from a program accepted by the Art Therapy Credentials Board, Inc. (ATCB);
  • Arrange for official documentation to be sent directly from the ATCB to the Health Licensing Office showing proof of holding a current credential as a Provisional Registered Art Therapist (ATR-P) by the date of application.
  • Have an established supervisory relationship and submit a Supervisory Declaration of Responsibility form which is available within the Provisional Art Therapist License Application;
  • Pass a fingerprint-based nationwide criminal records check; and
  • Pay all required fees.
