Option 1: Your health care provider, clinic or local pharmacy
Your medical provider or child's medical provider may print records from their medical record system or print the Immunization History Report from Oregon's Immunization Information System, ALERT IIS.
Option 2: Your child's school or child care
Your child's school or child care may print the Immunization History Report or Certificate of Immunizations Status (CIS) from ALERT IIS. Many schools and child cares in Oregon have access to ALERT IIS, Oregon's Immunization Information System.
Option 3: Oregon Immunization Program
Your age and where you have lived will affect how complete your immunization record is. ALERT IIS began collecting data for children in Oregon in 1996 and for adults in 2008.
