Food Service Regulations

Restaurant Forms
If you plan to operate a new or previously licensed restaurant, a Restaurant License Application must be submitted to the Local Public Health Authority in your area. Facilities that are being constructed or remodeled must complete the plan review process locally prior to construction. The license and plan review fees may vary, so contact your Local Public Health Authority for details.

Requesting a Variance to the Food Sanitation Rules
Restaurants must apply for a variance before starting any specialized processing, such as curing or vacuum-packaging foods as described in OAR 333-150-0000 Chapter 3-502.11. Please go to our Variance webpage for more information.

Mobile Unit Applications and Guidelines
If you plan to operate a new or previously licensed mobile unit, a Mobile Unit License Application must be submitted to the Local Public Health Authority in your area. Facilities that are being constructed or remodeled must complete the plan review process locally prior to construction. The license and plan review fees may vary, so contact your Local Public Health Authority for details.

Temporary Restaurant Packet

If you plan to operate a food booth at a public event, a Temporary Restaurant License Application or Review and fee (if applicable) must be submitted to the Local Public Health Authority for the county you will be operating in.

Benevolent Meal Site Guidelines
A Benevolent Meal Site is defined as a periodic food service operation run by a benevolent organization that provides food to the needy or indigent without charge and does not operate from a permanent kitchen facility.

Vending Machine Plan Review
This application only needs to be filled out if the vending machine does not have the sanitary approval of the National Automatic Merchandizing Association.  Please reach out to your Local Public Health Authority with any questions.
