About 

These trainings are for school staff and child care providers. The trainings cover immunization requirements and how to do the annual immunization reports.

Oregon Immunization Program Training Portal

To get ALERT IIS Access

1. Create an account for the OIP Training Portal, if you don't have one. 
   
2. Take one of the following trainings to completion (100%):
   
• Immunization Requirements, Reports and ALERT IIS for Schools and Child Care Webinar
  
• Using ALERT IIS for School and Child Care Course
  
• Usando ALERT IIS para escuelas y guardería curso
  
3. Read the Confidentiality Policy (every ALERT IIS user must read this document. (Español)
4. Read and sign the Authorized User Agreement (Individual School/Child Care Facility User) form. (Español)
   
5. For sites new to ALERT IIS or with new administrators, complete the Authorized Site Agreement (Colleges, Schools, Preschools, Head Starts and Child Care Facilities) form.  (Español)
   
6. Submit the following to the ALERT IIS Help Desk at alertiis@odhsoha.oregon.gov.
   
• Required: Training completion certificate
  
• Required: Authorized User Agreement (Individual School/Child Care Facility User)
  
• If Needed: Authorized Site Agreement (Colleges, Schools, Preschools, Head Starts and Child Care Facilities)
