The Transformation and Quality Strategy (TQS) was an annual CCO deliverable from 2018 to 2025 to move health system transformation.
Timeline

2018–2024: Annual CCO submission

2025: CCOs attested to meeting TQS requirements instead of submitting a full report. For details, see the
CCO contract waiver memo (2/5/25).
- 2026: TQS was removed from CCO contracts, with the focus shifting to Quality Assurance and Performance Improvement (QAPI) requirements and evaluation.

CCO submissions and OHA assessments
The projects included in each CCO’s TQS were a showcase of CCO work addressing TQS components. The work highlighted in the TQS was not a comprehensive catalogue or full representation of the CCO’s body of work addressing each component.
In 2025, CCOs attested to meeting TQS requirements instead of submitting a full TQS. See this list of
