Coordinated Care Organization Changes for Oregon Health Plan Members Covered by PacificSource in Lane County

  Oregon Health Plan (OHP) members in Lane County will continue to have their health care services covered.

What’s Happening:

PacificSource will no longer be a coordinated care organization (CCO) in Lane County in early 2026. OHP members in Lane County covered by PacificSource will have a new CCO: Trillium Community Health Plan (Trillium).

Oregon Health Authority (OHA), PacificSource and Trillium are working together to move OHP members to Trillium in early 2026. OHA understands you may worry about how this change affects you. OHA's goal is to make sure you keep getting the coverage and care you need.  

What This Means:

Current PacificSource CCO members in Lane County will automatically move to Trillium and continue to receive OHP benefits.

PacificSource is not taking new OHP members in Lane County.

What You Can Do:

If you are enrolled with PacificSource in Lane County, you will receive a letter at least 30 days before you move to Trillium.

Please be sure to keep your contact information current so that you get this letter. Here's how: Learn how to send changes to OHP.

The letter will say what to do to prepare. The move from PacificSource to Trillium will happen automatically.

This Does Not Change Your OHP Benefits.

Even if you change CCOs, your OHP benefits stay the same. You will not lose your health care.

PacificSource will serve current Lane County OHP members through the end of their contract. Please keep getting services as usual during this time. You can ask your health care providers if they accept Trillium. After you move to Trillium, Trillium will help you access the services you need.

When you move to a new CCO, you can still get care that was already approved. This is called a Transition of Care. It lets you keep getting those services for a short time, usually 30, 60 or 90 days, depending on your health needs and benefits.

About Current Health Care Providers:

Ask your current health care providers and pharmacy if they accept Trillium.

If they do, you don't need to do anything. You can keep seeing them.

If they don't, here are some steps you can take:

  1. Ask if they plan to join Trillium's network soon.
  2. If they won't take Trillium after you change CCOs, you might still be able to keep seeing them for a short time under OHP's Transition of Care rules.
  3. During this time, Trillium can help you:
    • Find in-network providers who meet your needs.
    • Make sure your care continues without interruption.

Transition of Care: What You Need to Know

If you change to a new CCO, you can still get care that was already approved. This is called a Transition of Care. It helps make sure your treatment continues without a break.

You can keep getting those services for a limited time. This is usually 30, 60 or 90 days, depending on your health needs and the type of care.

After the Transition of Care Period:

Once the transition of care period ends, whether you can keep seeing your current providers depends on if they are part of Trillium's network. To find out:

Trillium is working to add more providers to their network, so it's a good idea to keep checking if your provider isn't listed yet.

Some Members May Be Able to Ask for OHP Open Card Instead of Moving to Trillium.

This option is available at any time for members who have Medicare or are American Indian/Alaska Native.

In some cases, other members may qualify for temporary OHP Open Card if they have a serious health reason to keep seeing a provider who accepts OHP Open Card but not Trillium. You need to work with your provider to make the request.
