Getting and Reviewing Records

  1. Certificate of Immunization Status (CIS) form; the immunization record for children and students available in 17 languages

    The school and child care administrative rules(Español) were recently changed. A new CIS form that reflects this change is now available in English and Spanish. The new form will be available in additional languages in the upcoming weeks. Old versions of the CIS form are acceptable, with the exception that parent-verified history of varicella will no longer be accepted after August 1, 2025.

  2. ALERT IIS: Oregon immunization registry. A website to look up records and see if shots are due.
