This section provides information
necessary to complete ODE’s annual Medically Fragile/School Nurse Data Collection and Dental Screening Data Collection.
The annual Medically Fragile and
School Nurse data collection will be opening on May 7, 2020. This collection
asks for information on student health and school nurse staffing. Please review
the Medically Fragile Definitions and Guidance document and slides, as well as
the applicable Oregon statutes and rules before preparing and inputting your
data. Please submit your School District’s data by July 31, 2020. When you are
ready to input data, please visit the District Site.
The annual Dental Screening data
collection will also be opening on May 7, 2020. This collection asks for
information on student dental screening certificates. Please review the Data
Collection Guidance Slides as well as the Dental Collection FAQ document before
preparing your data. Please submit your School District’s data by July 31,
2020. When you are ready to input data, please visit the District Site.

your District
Security Administrator.
General Reminders:
These Data Collections are to be
submitted at the school district level only. ESDs and Charter Schools: please
work with School Districts to ensure that submission happens only once, from
the school district. Submissions should include all information from public
schools within the school district, including charter schools.
Changes due to COVID:
We know that the coronavirus
crisis has upended much of our daily work and yearly routines. With an
understanding that data collections may be especially challenging given
circumstances associated with school closures and physical distancing (and
everything else!), we’re extending the open windows for both the Medically
Fragile/School Nurse and Dental Screening collections for an extra month, from May
7, 2020- July 31, 2020.
Other 2020 Changes:
- Medically Fragile/School
Nurse: This year, how the list of student health acuity is ordered on the
submission page has changed. The submission fields are the same, just the
ordering has changed to reflect increasing acuity. The PowerPoint slides
provide a screenshot of how this looks.
- Dental Screening: This
year, this data collection will be completed on the ODE District Webpage,
along with most other collections. This will no longer be submitted via
Smart Sheets. Also, please note that the submission fields are asking for
the same data as prior years— but we’ve clarified some of the language.
