Confidential Income Statements are the Oregon Department of Education required forms which Family Day Care Home Sponsors must use to collect information from households to determine their eilgibilities for Tier 1 and Tier 2 Meals.
When Sponsors distribute the current Confidential Income Statement they must also distribute the Letter to the Household at the same time.
