The Annual Application is used by school districts, EI/ECSE's and programs to assure they will operate consistent with the requirements of Individuals with Disabilities Education Act (IDEA).
All districts are required to submit a completed CEIS form annually in the fall, and if necessary, make corrections by April 1 even if IDEA Part B funds are not used for Coordinated Early Intervening Services. Coordinated Early Intervening Services may be voluntary or required if the district was found to be significantly disproportionate.
All districts are required to submit a completed Private School Data Form annually in the fall. The form collects 1) private school enrollment data and 2) calculates proportionate share of IDEA Funds for parentally placed private school students.
